What to bring to your appointment:

• Photo ID (e.g. Driver's License)

• All Medical Insurance Cards (Medicare, Commercial Plans, etc.)

• Out-of-pocket financial responsibilities

  • copay, deductible, co-insurance, glasses refraction, etc.

• Current glasses or contact lenses

• Current list of medications

• Medical records from referring doctor (if applicable)

What to expect at your appointment:

• Appointment time: New appointments usually take 1-1.5 hours, including check-in, examination, any required testing, and counseling by Dr. Kim.

• Dilation: As part of a complete eye examination, your eyes will be dilated unless medically contraindicated. Because driving may be difficult immediately after an examination, you may want to make arrangements not to drive yourself.

• If the patient is a minor (under 18 years old), a parent or legal guardian must be present at the initial appointment.
